Case Description: Pt is a 64 y/o female who arrived to physical therapy with reports of pain in the front of the left hip after bending over to water her plants. She has difficulty with tasks requiring bending over including feeding her dogs and letting them out, self care such as dressing, as well as sit to stands, stairs, and prolonged weight bearing.
Objective Measures: The patient was seen for physical therapy 10 visits over a course of 1.5 months.

Summary: The patient was discharged from physical therapy due to reports of 99.9% improvement since the start of therapy; reports no pain or difficulty with performance of her required daily activities. She was educated to continue with the performance of her HEP to maintain gains in strength and ROM as well as tolerance to daily activities.
