The first step in starting your physical therapy program is to complete the necessary paperwork. We realize the time you have available for physical therapy is limited. To assist in your time management, you can download the New Patient Forms listed to the right and complete them before you arrive for your first appointment. Please put your name on each form.

Many new patients ask what to wear to their first visit. Comfortable, loose-fitting clothes are best, especially those with elastic waistlines. Those patients who have a hip or knee condition should bring a pair of shorts (we do have some in stock for you to borrow if necessary).

Although we cannot always accommodate your scheduling needs, we make every effort by offering early and late appointment times and we do our best to keep on schedule. As a courtesy to us, we ask that you be on time and provide 24 hours notice if you must cancel your appointment. A CANCELLATION FEE OF $50 MAY BE ASSESSED IF 24 HOURS NOTICE IS NOT GIVEN! Being late for your appointments and inconsistent attendence are two main reasons why a person does not improve as expected.
